Move your teeth versus an implant?

ScienceDaily (Oct. 10, 2011) — Although replacing lost teeth often involves artificially building up the jaw, researchers at the Sahlgrenska Academy at the University of Gothenburg, Sweden, are now showcasing a new method whereby teeth are instead moved into the toothless area using a brace, giving patients the chance of having more teeth.


When we lose our teeth, perhaps because of illness or injury, the jaw in the toothless area also decreases in volume. This reduction makes it difficult to carry out dental implants, often leaving just one option for replacing lost teeth: building up the jaw with bone transplant.

Alternative method

Researchers at the University of Gothenburg’s Sahlgrenska Academy are now presenting an alternative method. In an experimental study on dogs, the Gothenburg researchers managed to use a brace to move existing teeth into a toothless area with limited bone volume, without any reduction of the tooth’s natural attachment in the jaw.

In a subsequent clinical study, consultant Orthodontist Birgitta Lindskog Stokland and her colleagues also managed to show that the same procedure in humans caused only small changes in the tissue around the tooth.

No lasting problems

“X-rays showed some damage to the root known as root resorption, but this didn’t seem to cause any lasting problems,” says Lindskog Stokland. “What’s more, our follow-ups a year later showed that the damage had lessened.”

The original site of the moved tooth suffers a reduction in bone mass and dental tissue volume, though not to the same extent as when teeth come out for other reasons. This means that this area is well-suited to implants or other tooth replacements, without there being any need for bone transplants.

More teeth more easily

“In other words, many patients can be given more teeth more easily,” says Lindskog Stoklan.

A dental implant is an option to replace a missing tooth. A small titanium shaft is surgically implanted into the bone and allowed to set. The bone grows around it forming a tight connection, which additionally slows or stops the bone loss that occurs when the root of a natural tooth is missing. Once the implant is firmly set in the mouth, Dr. Delaney then works to attach the replacement tooth onto the top
of the shaft. This permanent solution has the
advantages over bridge work that it does not stress the surrounding teeth for support, and, should the tooth wear out, another can simply be replaced on the shaft.

Implants can also be used as support as part of an implant bridge. This is an alternative to partial dentures, and has several advantages. First, there is no adjustment period to acclimatize the patient who, once the work is done, only feels teeth, not metal supports intruding into the mouth. Second, this slows
the bone loss occasioned by missing teeth. Third, there is no discomfort or difficulty in eating. And, best of all, they don’t have to be taken out all the time.

Bridges

Bridges are used to fill the space created by a missing tooth. It is formed to look like the missing tooth, and it takes its place in the mouth. The sides of a bridge use the two surrounding teeth for support. A bridge replaces the missing tooth, both functionally and cosmetically. Bridge work is as much an art as it is an exact science. The materials used
may be gold alloys, porcelain bonded to metal alloy, or all ceramic material. The choice of material depends on requirements for strength, wear, and/or esthetics.

It is important that a missing tooth be replaced as soon as possible for several reasons. If not treated the teeth surrounding the gap begin to shift inward, creating a whole chain reaction of bad things. Teeth use their neighbors for support, and, with one missing, they start to “fall.” The bite changes in response to the pressure. This can eventually result in problems with the entire jaw, e.g. TMJ. The

surrounding teeth deteriorate and it is just a matter of time before they are lost. Gum disease becomes a serious problem, with the difficulty of treatment increasing as the neglect continues.

Coffee, tea, tobacco, red wine, antibiotics and age can all darken teeth. Patients can choose from two routes to whiter teeth: at-home whitening or in-office whitening.

• At-home bleaching kits use carbamide peroxide solutions to oxidize and bleach your teeth. Your dentist will create customized trays for your mouth based on plaster molds of your teeth. Place the solution in these trays, then hold the tray in your mouth for the amount of time specified by your dentist. He or she will determine a duration of treatment. In a few weeks, return to your dentist for a follow-up appointment.

• In-office bleaching uses hydrogen peroxide gels and a heat or light source, these treatments lighten your teeth by six to eight shades in one to three visits, and sometimes less. The light source
